How to Refer a Patient
1) Your patient can schedule a free 20 minute virtual meeting or call for information, to see if I can help. Just provide my information.

How it Works
Seamless transition to support beyond your office visit
Step 1: Identify the Patient
Refer patients who may need additional support with follow-through, lifestyle change, GLP-1 medication support, metabolic health goals, or care plan implementation.
Step 2: Direct the Patient to Schedule
The patient schedules directly through the coaching website or referral link. No insurance billing or administrative setup is required from your office. Our first meeting is just 20 minutes to do a quick Q and A to see if my support is right for them and how I can help.
Step 3: Patient Completes Intake
The patient completes a brief intake form to share their health goals, current provider recommendations, medications, symptoms, challenges, and priorities.
Step 4: RN-Led Coaching Begins
Coaching focuses on education, accountability, habit change, visit preparation, side effect awareness, and helping the patient understand and follow the plan already recommended by their provider.
Step 5: Patient Remains Under Provider Care
All diagnosis, prescribing, medication changes, and treatment decisions remain with the patient’s licensed medical provider.
I support them from this visit through follow-up visits, and for as long as they need me. I'll also help prepare them for their provider follow up appt.

A basic 12-week program - x4 , 45 min weekly sessions to review the AVS, educate your patient, decide priorities, create and install small new habits, and routines, followed by bi-weekly (once every 2 weeks) 30 minute check in sessions for support, progress review, accountability and adjustments - total 8 sessions. Ideal for new lifestyle modification for health, like GLP-1, managing blood pressure, preventative health and wellness.
All plans are customizable and flexible based on patient needs.
